Specialist Support Coordination
For participants with more complex needs, we offer Specialist Support Coordination, a higher level of support to manage challenges in your support environment and ensure consistent service delivery.
What you can expect from us
You'll have a dedicated coordinator who knows you and your plan, responds promptly, and keeps you informed at every step. We document conversations clearly, send regular progress updates, and never make decisions about your supports without your input.
Our coordinators are independent in their advice, we'll recommend the providers that best fit your goals, not just the ones we know. Conflict-of-interest disclosures are part of how we work.
Crisis and complex situations
When things go wrong, a hospital admission, a sudden change in housing, a breakdown in supports, we move quickly. We help you stabilise the situation, coordinate emergency supports, advocate with the NDIA and your providers, and build a plan to prevent the same thing happening again.
Preparing for your plan review
We help you reflect on the past plan period, gather evidence from providers and therapists, and build a clear, well-supported case for the funding and supports you'll need going forward. Plan reviews shouldn't be stressful, with preparation, they're a chance to set yourself up for the next year.

We can, but we're transparent about choice and control. You are never required to use Wise Healthcare for your other supports.
We agree a contact rhythm that suits you, usually monthly check-ins with more contact early in your plan or during transitions.
